THE WORK
01The challenge:
Reminders were a front-desk chore, and one visit in six was missed
MedConnect runs nine outpatient clinics across two states, and every appointment went into a scheduling tool that sent nothing on its own. Front-desk staff spent their afternoons phoning tomorrow's patients, and insurance was checked by hand on the morning of the visit, often after the patient had already sat down.
Roughly one booked visit in six ended as a no-show. Each empty slot was a clinician paid to wait, and patients who tried to cancel usually reached voicemail, so the slot stayed empty even when someone on the waitlist would have taken it that afternoon.
02What we built:
A scheduler that confirms, reminds and checks insurance on its own
Patients book online or at the front desk into a Next.js scheduler on PostgreSQL that holds every clinic's providers, hours and visit types. Twilio texts a confirmation at booking, a reminder three days out and another two hours before. Replying C confirms, R opens a reschedule link, and telehealth patients get their video link in the same thread.
When a patient cancels, the slot reopens and the first person on that clinic's waitlist gets a text offering it. Eligibility is checked through a clearinghouse API two days ahead, and failures go to a front-desk queue. The Node.js services run on AWS with encrypted storage and a full audit log, which HIPAA required.
03What changed:
No-shows fell 60%, and the front desks stopped making reminder calls
No-shows fell 60% across the nine clinics, from about one booked visit in six to roughly one in fourteen. Slots freed by late cancellations now go to the waitlist instead of sitting empty, and insurance problems surface two days ahead rather than at the check-in window.
Each front desk got back about two hours a day that used to go to reminder calls. We keep the platform on a monthly retainer, covering Twilio templates, AWS patching and each new clinic, and MedConnect's operations team sets visit types and reminder timing from an admin screen.
